The Health Care for the Homeless Case Manager/Outreach Worker informs FHC patients, community members, and community partners of FHC services/programs, provides general health information, and represents FHC at community and enrollment events. Demonstrates expertise in FHC services/programs, the needs of the underserved and vulnerable populations; privacy and security standards; general health information; and insurance enrollment facilitation. Conducts public education activities to raise awareness about FHC services and programs, general health information and procedures in a fair, accurate, and impartial manner. Provides information and assistance in a manner that is culturally and linguistically appropriate to diverse communities and accessible to individuals with disabilities; and provides referrals to any applicable office of health insurance consumer assistance or ombudsman established under Section 2793 of the PHS Act to address consumers’ grievances, complaints, or questions about their health plan, coverage, or a determination. HCH CM/Outreach Worker must maintain a professional, collegial positive relationship with staff and clients. Demonstrated ability to effectively engage with high-risk population and work with a low-income, multi-ethnic client population and demonstrate ability to relate respectfully and effectively to people of diverse racial, educational and socio-economic backgrounds. Must have strong communication, organization, documentation and record keeping skills, and be a confident, self-directed individual who can work within a team as well as independently. Current, unrestricted driver’s license required. Peer Support Specialist, computer skills and bilingual English/Spanish preferred.
