Cowlitz Family Health Center (CFHC) is located between the Columbia and Cowlitz Rivers, Mt. St. Helens and the Pacific Ocean in southwest Washington.  CFHC serves over 25,000 people in its 10 outpatient clinics, 2 residential treatment facilities and 3 social service sites.   With over 250 health professionals and support staff and a budget of $27 million, CFHC provides access to quality medical, dental, family planning, W.I.C., First Steps maternal health services, substance abuse treatment, medication assisted treatment, harm reduction (syringe services) and enabling services which includes outreach, eligibility assistance, case management, transportation and interpretation services.  CFHC serves the whole community including low income, homeless and other individuals with barriers.
Our Community Partners About Our Service Area
FHC partners with other organizations to improve the health of our community. FHC has an RN and a Substance Use Disorder Counselor rounding at the hospital in Longview to provide transitions of care and to engage patients in care after they leave the hospital. FHC is a pilot location for collection of social determinants of health information and helping patients access community resources. FHC partners with Love Overwhelming for outreach to homeless individuals to connect them to health care. FHC partners with Community House/CORE Health, Goodwill, schools and others to provide health education. FHC partners with PeaceHealth and Lower Columbia CAP on a variety of projects, including Care to Home – a project to provide respite care for vulnerable individuals leaving the hospital who need temporary supportive housing. FHC provides case management support for Phoenix House, a joint permanent supportive housing project between FHC and Housing Opportunities of SW Washington.

About CFHC’s Service Area

CFHC serves Cowlitz, Wahkiakum, Pacific and northern Clark counties in the State of Washington.  These counties are located in the far south west corner of Washington State and make up 3,163 square miles.  The area is mixed urban and rural and home to 285,935 people.  The majority of the population resides in Cowlitz County.  Pacific and Wahkiakum counties are very rural.  The Columbia River separates the southern edge of the area from Oregon.  The area is bordered by the Pacific Ocean to the west and the Cascade Mountain Range to the east.  Interstate 5 connects the area to Portland and Seattle.  Fishing, logging, manufacturing, education, health care, construction and service industries make up the bulk of the economy.  Cowlitz County was once known as “Timber Capital of the World.”  Since 1980 and the eruption of Mount St Helens, tourism plays a larger part in the economy. 

Cowlitz County is home to Kelso and Longview, a thriving set of communities with many events, tourist opportunities and shopping options.  The Lower Columbia College is in Longview and offers both traditional 2-year AA degrees as well as a variety of 4-year options.  Cowlitz is also the home of Mt. Saint Helens, truly one of the amazing natural features in the entire US.  Woodland Washington is in both Cowlitz County and northern Clark County.  Woodland is at the southern gateway to Mt. Saint Helens.  Moving west along the Columbia River, Wahkiakum County features one of the earliest settlements in the state.  It is also home to the last scheduled ferry crossing the Columbia River to Westport Oregon.  Then, to the western most part of Southwest Washington, we find Pacific County located at the mouth of the Columbia and along the Pacific Ocean.

Our region features a wide variety of events, concerts, fishing, boating, kayaking, as well as hiking, biking, and camping.  There are farmers markets, a flower market, crafts fairs, and art galleries.  Because of the rural character of this region, there are also numerous historic buildings and barns.   Some of these historic properties have been repurposed as venues for weddings, parties, and other gatherings.  Pacific County offers the visitor miles of white sand beach up the Long Beach Peninsula. Outdoor recreation is a way of life in Southwest Washington.
