2. What should I bring to my first appointment?
    
You should bring picture ID, your insurance cards or medical coupons, immunization records, previous medical records (if you have them), and a list of any medications you take. If you are not insured, please bring $15 (medical) or $25 (dental). This amount will be applied to your charges. You will be asked for your current address and telephone number.

Our Longview,Woodland, and Wahkiakum Clinic providers use St. John Medical Center in Longview. Physicians from PeaceHealth Medical Group assist us in serving our hospitalized patients in Longview.
 
Our North Beach Clinic physician uses Ocean Beach Hospital in Ilwaco. Physicians from Ocean Beach Clinic assist us in serving our hospitalized patients.

4. What if I need a specialist?
    
A referral is made by the patient’s primary care provider and For insured patients, the cost is paid through your insurance plan. For uninsured patients, our staff will try to help the patient with the care they need at an affordable cost.
