- North Beach Clinic (21610 Pacific Highway, Ocean Park) provides Medical, Dental and Outreach services.
- Woodland Clinic (1251 Lewis River Road, Woodland) provides Medical, WIC and Outreach services.
- WIC has a satellite clinic at Castle Rock Grange.
- FHC stations an outreach nurse at the Kelso DSHS office to provide family planning.
Evening and Saturday hours are available at our Longview Clinic. Our clinicians are available after hours 7 days a week for urgent health questions.

PAYMENT for SERVICES
FHC accepts Medicaid, Medicare, Community Health Plan of Washington Healthy Options and Basic Health Plan, Molina, most private health insurance, and private pay. Co-pays and deductibles are due at the time of service. FHC accepts payment by cash, check, money order, VISA, Master Card, or debit card.
For the convenience of uninsured patients with limited family income, FHC offers a SLIDING FEE. Sliding fee discounts are based on family gross income and size. FHC staff will assist patients with the application. FHC staff assists those with no health insurance to apply for Medicaid or the Basic Health Plan.
No one is denied required Family Planning or Urgent Primary Care services because of an inability to pay. Patients are responsible for discussing the circumstances of inability to pay or the need for a payment plan with FHC staff.
